Job Profile Summary

Under the supervision of the Vice President PACE and Older Adult Services, the RN Manager of PACE Home Services oversees the day-to-day operations of the Home Care Services for PACE Program across all PACE Centers. This includes home care (unskilled) and home health (skilled) services provided directly by FHCSD employees and contract employees. The position also oversees the provision of home care (skilled) services provided under contract or directly by FHCSD employees. The RN Manager of PACE Home Services provides direct supervision to Home Care Coordinators (RN's, LVN's) and the Certified Nursing Assistants (CNAs), Personal Care Attendants (PCA), providing home care services to PACE participants. The position will work with PACE leadership to complete and submit all necessary components of the application to obtain a Home Health License for the organization. The RN Manager of PACE Home Services will then help to build out the Skilled Home Health services including staffing and supervising the staff in this department. The position provides supervision visits to home care staff in the field to evaluate skill and performance. The position assists PACE leadership in policy formulation and program development and implementation. The RN Manager of PACE Home Services ensures home health and home care services are provided in accord with the Care Plan. This position is responsible for ensuring compliance with annual budgets and financial objectives, company policies and procedures, and guidelines. The position ensures timely completion of Home Care initial, semi-annual and change of condition/address assessments to meet any policy and regulations. The position acts as the liaison between any contracted home health agencies and FHCSD-PACE and ensures compliance with staff competency requirements. The position ensures all patient encounters are documented in the medical record.

Job Roles

  • Oversees the supervision of the CNAs and PCAs providing home care services ensure adequate coverage, deployment of staff, staff efficiency and documentation of all encounters for the FHCSD PACE Centers.
  • Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing the care plan.
  • Monitors patient care to ensure quality and appropriateness of home care and home health services for the FHCSD PACE Centers.
  • Oversees the Skilled Home Health Staff, RNs and LVNs, once the Home Health License for the organization has been approved for the FHCSD PACE Centers.
  • Ensures RN and LVN Home Care Coordinators staff perform initial, semi-annual, change of condition/address assessments of participants in their home environment to determine their individual needs and ensures coordinated plan of care with care giving resources for the FHCSD PACE Centers.
  • Conducts supervised visits to home care staff in the field to evaluate skill and performance.
  • Maintains effective interdepartmental communication.
  • Ensures home health and home care services are provided in accord with the Care Plan.
  • Responsible for ensuring compliance with annual budgets and financial objectives, company policies and procedures, and guidelines.
  • The position acts as the liaison between any contracted home health agencies and FHCSD-PACE and ensures compliance with staff competency requirements. The position ensures all patient encounters are documented in the medical record.
  • Completes orientation, initial and annual competencies, supervises and evaluates performance of LVN Home Care Coordinators, CNA's, PCA's, and Home Health RN's and LVN's.
  • Collaborates with other nursing leaders in the organization and Center Managers and PACE Leadership on best practices and in home services policies and program development.
  • Perform data collection, analysis, and reporting as required.
  • Assist PACE management in preparation for program audits.
  • Works with FHCSD leadership to prepare for and obtain a Home Health License for the organization.
  • In collaboration with PACE management team, coordinate and facilitate all State and federal rules on compliance updates and implementation related to the overall integrity of the quality assurance program.
  • Perform other duties as assigned.

Education/Certifications/Licenses/Registrations

  • California State Board RN License required.
  • Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training.
  • Graduation from a recognized RN/BSN program.
  • Ability and means to travel as needed in a timely manner within San Diego County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Licensed for a minimum of 3 years; No more than 2 violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, or reckless driving within 7 years.
  • Professional certification is preferred.

Experience/Specialized skills (including Language)

  • Broad knowledge base of health care problems of the frail elderly.

  • Familiarity with community resources available to participants and families.
  • Skilled in developing and maintaining department quality assurance.
  • Skilled in establishing and maintaining effective working relationships with participants, family caregivers. Interdisciplinary Team, and other staff members.
  • Ability to react calmly and effectively in emergency situations.2 years of experience in compliance or quality assurance.
  • Demonstrated experience in performing tasks related to quality assurance processes, data collection, analysis, and/or systems analysis.
  • Strong analytical, organizational, and interpersonal communication skills (written, verbal and presentation).
  • A minimum of one year's experience in working with frail or elderly population required.
  • Demonstrated experience preparing comprehensive, accurate, and detailed analyses.
  • Technical proficiency with the Microsoft Office suite of applications.
  • Experience identifying and presenting unfavorable or controversial information with tact and skill.
  • Experience working with accreditation or regulatory standards highly desirable.

101,346.00 - 154,885.00

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
