The Bariatric Outreach Coach position is responsible for the clinical coordination of care and services for UBH members who are pursuing Bariatric surgery. This position serves a critical function as a member of the Bariatric Outreach team. Bariatric Outreach Coaches work closely with members over an extensive period of time to educate about needed lifestyle changes to make the surgery a success. The Coach/Care Advocate engages members in setting and keeping specific goals to support their long term success. Behavioral health screening, education, referrals, relapse prevention and coordiantion of care are key components of this role.
Primary Responsibilities include:
  • Referral to UBH provider for pre-surgery psychological assessment and review of assessment results to determine barriers to surgery
  • Telephonic member outreach to review the assessment results and offer appropriate services and referrals.
  • Coordination with referring partner regarding readiness for surgery
  • Monthly telephonic coaching sessions with member to engage them in their recovery by educating about success factors
  • Offering referrals to clinicians and/or other resources, setting specific goals and reviewing progress towards those goals
  • Regular case review with supervisor and medical director to address issues and concerns
  • Documentation and activity tracking in various systems in accordance with UBH policies
  • Active participation in staff meetings, case conferences and in-service opportunities
  • Back-up coverage to other clinical programs as needed.

  • Encouraging involvement in pre and post surgery support programs

Qualifications
Required:
  • One of the following is required: Registered Psychiatric Nurse, Licensed Master's Level Mental Health Professional or Licensed PhD with Medical or Health Psychology background.
  • Licenses must be current and unrestricted for the state of California.
  • Minimum 3 years POST LICENSURE direct clinical experience is required.
  • Ability to identify and prioritize member needs, in addition to providing structured and focused interventions.
  • Excellent communication skills.
  • Creative problem solving skills with the ability to use community and network resources to meet members' needs.
  • Excellent time management and prioritization skills.
  • Must be able to work fairly autonomously.
  • Team oriented.
  • Strong computer skills, must be able to type and talk at the same time, in addition to navigating around a Windows environment.
Preferred:
  • Experience with outpatient and community based treatment of behavioral health, substance abuse and/or medical comorbidities is strongly preferred.
  • Knowledge/experience in management and treatment of morbid obesity is preferred.
  • Previous experience with Bariatrics is strongly preferred.
  • Behavioral health coaching experience is preferred.
  • Experience working with individuals with a high relapse rate is strongly preferred.

Nice to have:
  • Previous experience in a telephonic role.
