LifeLong  Medical Care is seeking  candidates for our Advanced Practice Provider Fellowship Program for Adult Gerontology Nurse Practitioners, Family Nurse Practitioners, and Physician Assistants in Community Health. 12 -month program (5 days a week), graduated onboarding, dedicated preceptors, and didactic sessions.

 

Our program is a 12-month intensive training program for new FNPs, AGNPs, PAs who have recently completed their Master or Doctorate in Nursing,  or Physician Assistant program within the last 18 months; our program consists of primary care clinics with dedicated preceptors, specialty rotations, weekly didactic training, and curriculum focusing on community health issues through a health equity lens and DEI and anti-oppression framework, and health equity curriculum project.

 

 

Benefits:

Responsibilities

Program Goals

 

  • To increase mastery and confidence for new APPs in assuming full responsibility for the primary health care of complex underserved populations across the life cycle and in multiple settings.
  • To support the transition of new APPs to proficient primary care providers by building upon the education base already completed, and, using precepted clinical experience and mentoring, to develop the clinical confidence necessary for efficient, effective and productive practice as a member of the health care team in a FQHC.
  • To increase the number of Advanced Practice Providers choosing to build long-term careers in FQHCs, and preparing to take on leadership roles within those organizations and within the healthcare system of the future.

 

Qualifications

Applicants must meet the following qualifications to become eligible to apply:

  • Completed MSN, DNP, or PA program within the last 18 months
  • California state RN & NP or PA license by the start of the program
  • Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, or Physician Assistant certified by an accredited agency by the start of the program
  • Demonstrated commitment to working in a safety-net setting
  • Bilingual (especially Spanish) required for some positions

While performing the duties of this job, the employee is frequently required to stand, walk, and sit; use hands to finger, handle, or feel and reach with hands and arms; talk and hear. The employee must occasionally lift and/or move up to 25 pounds.  The employee frequently works with a video display terminal for prolonged periods; vision abilities include close vision and ability to adjust focus. 

 

While performing the duties of this job, the employee regularly works in inside environmental conditions.  The employee may be required to attend evening meetings, travel and work evenings or weekends.  The noise level is usually quiet. Individual is subject to frequent interruptions.

   

Supervisory Responsibility 

        

This position has no direct staff supervisory responsibilities
