Responsibilities

Horizon Health, the nation's largest operator of hospital based behavioral health services, in partnership with Yuma Regional Medical Center in Yuma, AZ. is seeking an IOP Coordinator to provide oversight of the Intensive Outpatient Program (IOP) at Yuma Regional Medical Center.

Essential Core Functions:

  • Responds to intake calls and completes admission paperwork. Assessments will be completed by RN, LPC or LMSW
  • Facilitates groups, family and individual sessions and completing all necessary documentation
  • Schedules Unit assessments, meetings and appointments as appropriate
  • Utilizes assessment tools including, but not limited to Geriatric Depression Scale, Zung, SLUMS, alcohol screening tool(s) under authorization/supervision of RN, LPC or LMSW
  • Communicates with referral sources including, family members, legal system, psychiatrists, primary care physicians, and other treatment facilities.
  • Reviews newly admitted patient accounts for eligibility and authorization daily
  • Maintains schedule for filing court paperwork and hearings. Works closely with court personnel to maintain good relationships
  • Participates as an active member of the interdisciplinary team
  • Addresses issues in a timely manner including crisis and higher level of care referrals.

Qualifications

Education, Certification/Licensure, and/or Experience

  • A Bachelor's Degree from an accredited college with a major in Social Work, Psychology, Sociology or similar field
  • A Master's Degree or above from an accredited college with a major in Social Work (MSW)
  • Must have a valid state license in Social Work as required by the state in which the Social Worker will be providing services (LMSW or state equivalent)
  • Minimum three years' of experience in a similar position and/or industry
