POSITION SUMMARY

The Nurse Practitioner (NP) is part of a multi-disciplinary team whose primary purpose is to determine, assess and plan public health and case management services, ensuring that services promote early detection of diseases or illnesses, while emphasizing health education and health services that are preventive in nature. A major focus of the NP's responsibilities will be Women's Healthcare. The incumbent will represent United American Indian Involvement, Inc. (UAII) at community events, meetings, and during program reviews.

ESSENTIAL DUTIES, FUNCTIONS & RESPONSIBILITIES

  • Assess, diagnose, and treat the healthcare needs of women throughout their lifespan.
  • Provide contraceptive counseling and care including placing intrauterine contraception and diaphragms.
  • Provide prenatal care.
  • Counsel and provide care before and after menopause.
  • Evaluate and treat common infections.
  • Provide health screening for risk assessment of adults and children dealing with general medical, chronic diseases and HIV/AIDS risk assessment and risk reduction.
  • Provide referrals for diagnosis and treatment to assure continuity of health services.
  • Responsible for achieving a percentage of encounters & services for face-to-face evaluations, education & screening as well as home visits as required by Indian Health Services or other funding sources.
  • Responsible for completing a percentage of chart reviews for quality assurance purposes as assigned by the Healthcare Clinical Operations Director.
  • Maintain an active list of all clients sorted by geographic area (to facilitate home visit scheduling).
  • Provide immunizations and assist with schedules.
  • Provide health and wellness counseling as appropriate to all clients.
  • Adhere to and track Government Performance and Results Act (GPRA) performance indicators related to the delivery of services.
  • Refer clients to appropriate primary and specialty care in partnership with case management staff.
  • Submit required monthly reports and records as required by Healthcare Clinical Operations Director.

  • Ensure appropriate treatment plans and continuity of care with case management and professional staff.
  • Responsible for program goals, plan enhancements and improved outcomes.
  • Maintain documentation of all encounters utilizing ICD-9 and CPT codes for data management system.
  • Participate in weekly Clinical meetings.
  • Adheres to organization's policies and procedures and advances the goals of UAII in a manner that embodies the organization's philosophy.
  • Keep Director well informed regarding activities, pending issues and potential problems.
  • Other duties as assigned or required to fulfill the purpose of the position.

MINIMUM QUALIFICATIONS

Qualifications:

  • Ability to work as part of a multidisciplinary team.
  • Ability to work with professional and lay volunteers.
  • A self-starter who can work independently and as a team member; who consistently demonstrates professionalism, courtesy, efficiency, excellent internal and external customer service, high ethical standards and behavior that contributes to harmonious relationships.
  • Demonstrated ability or willingness to work with culturally diverse populations, youth, and professionals.

Education & Experience:

  • Master's degree in nursing from an accredited University
  • NP Program Completion approved by CA state Board of Nursing

Certifications:

  • Current California Registered Nursing License
  • Current Nurse Practitioner Certificate
  • Current Basic Life Support (BLS) Certificate
  • National Nurse Practitioner Certificate
  • Womens Health Nurse Practitioner (WHNP) Certificate

Confidentiality:

  • Maintain strict confidentiality of client and personnel information adhering to HIPAA, professional codes of conduct, State of CA regulations, Privacy Act, and UAII policies and procedures
  • This position has access to sensitive information, and a breach in confidentiality will be grounds for disciplinary action up to and including immediate termination

Communication/ Interpersonal Skills:

  • Possess the ability to communicate with others in a courteous and professional manner
  • Excellent customer service skills
  • Proficient in English

Technology Skills:

  • Working knowledge of Microsoft Office Suite (Word, Excel, PowerPoint)
  • Working knowledge of Google Suite (Mail, Calendar, Drive)
  • Working knowledge of virtual video platforms (Zoom, Google Meets)

  • Demonstrate ability to exercise a high level of judgment, accept supervision, and exercise a high level of accuracy and organization
  • Available and willing to travel as needed
  • Available to work evening and weekend hours as needed
  • Willingness to learn new skills and participate in training sessions relevant to the position
  • Maintains personal responsibility for maintaining a valid licensure and for ensuring timely completion of appropriate Continuing Education Units for renewal of license

POSITION PREFERENCES

  • Bilingual Spanish/English
  • Training and/or experience performing colposcopies and cervical biopsies
  • Experience providing STI screenings and counseling
  • Experience working with the American Indian/Alaskan Native community
  • Experience working with nonprofit organizations, community groups, and performance-based government contracts.
  • Knowledge of Indian Self-Determination and Education Assistance Act (Public Law 93-638)
  • At least 2 years' experience working in the public health field.
  • At least 2 years' experience working with women's care, or
  • At least 2 years' experience working with vaccinations and schedules, or
  • At least 2 years' experience working with acute out-patient care settings.
  • Women's Health Nurse Practitioner (WHNP) Certificate or equivalent work experience

Physical Demands:

The physical demands described here represent those that an employee must meet to perform the primary functions of this job successfully. While performing the duties of this job, the employee is required to stand, walk frequently, sit, bend, twist, talk, and hear. There are prolonged periods of sitting, keyboarding, reading, as well. The employee must occasionally lift and move up to 20 pounds. Specific vision abilities required by this job include reading, distance, computer, and color vision. Talking and hearing are essential to communicate with staff and vendors.
