Pay Rate: $45 - $62 depending on experience

Responsibilities

Position Summary

Oversees the coordination of all clinical documentation required for completion of the Inpatient Rehabilitation Facility Patient Assessment Instrument (IRF-PAI), and provides oversight regarding the length of stay in collaboration with the clinical team. Conducts ongoing review of the CMS data submitted and the patient's medical record to assure that the documentation is accurate and that the data is congruent. Supervises PPS Coordinator(s) in performing PPS related job functions.

Specific Duties

  • Reviews all documentation for submission to the Centers for Medicare and Medicaid Services (CMS), confers with the clinical team, and directs the correction of documentation on the IRF-PAI when indicated.
  • Assesses clinical documentation for missing or errant entries and notifies clinical leadership or clinicians for corrections or addendums.
  • Verifies the accuracy of the information contained on the IRF-PAI and the electronic medical record and billing software.
  • Assures timely submission of all IRF-PAI records to CMS, and coordinates the timely correction of IRF-PAI corrections.
  • Reviews all co-morbidities on the IRF-PAI and in the Medical Record to assure that co-morbidities are properly captured and documented in both locations. Confers with the Medical Records Manager to assure proper coding according to coding conventions.
  • Reviews all current patients in house daily in collaboration with the clinical teams to assure proper progress along the critical pathway. Provides the team members with correct information regarding the targeted length of stay.
  • Provides feedback to the hospital CEO regarding the patients currently in house and their status with regard to the targeted length of stay.
  • Works closely with the Director of Case Management regarding utilization review both concurrently and retrospectively, and participates in any site-specific utilization review activities.
  • Reviews Medical Records, both concurrently and retrospectively, tracts trends and data, and provides a report and feedback to the Facility Management Team regarding documentation.
  • Works directly with any clinical department to focus on documentation trends and corrective action to assure that the Medical Record documentation supports the IRF-PAI documentation.
  • In collaboration with Program Outcomes Coordinator, works to ensure that outcomes data and scoring is consistent among all facilities and is accurate according to national standards and guidelines for proper scoring.
  • According to need, provides coverage at other sites for PPS managers who are unavailable.
  • Confers with the patient and clinical team members regarding the scoring of FIM items, follows up on any inconsistencies, and, when necessary, assesses the patient to confirm the correct score.
  • Reviews all Rehabilitation Impairment Categories and admission ICD-9 codes to assure congruence and proper Coding for the IRF-PAI. Makes any necessary changes in collaboration with the clinical team and physician.
  • Performs other duties as assigned.
Qualifications

Minimum Qualifications

  • Current state licensure as a Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT) or Speech Language Pathologist (SLP) required.
  • Minimum of two years experience in rehabilitation or a closely related field required.
  • Experience as a Case Manager in rehabilitation preferred.
