CSI understands clients and staff have been impacted by trauma. CSI is becoming a trauma-informed organization by hiring individuals with existing experience in trauma-informed practices as well as supporting ongoing staff development with trauma-informed practices. This will include learning to recognize symptoms of trauma and how to respond, through training starting at orientation and throughout their career at CSI. Program Overview

Community Solutions, Inc. is seeking a dedicated and passionate candidate to fill the role of Case Manager for Ventura Case Management located in Ventura, CA. The general case management duties will include but not limited to intake, assessment, case record management, and discharge preparation as well as facilitation of evidence based curricula.

Ventura Case Management provides evidence-based case management to Ventura County Probation Clients. The program address the fundamental principle of evidence-based correctional practice by: objectively assessing- criminogenic risks, enhancing intrinsic motivation, targeting higher-risk offenders, and addressing offenders' greatest criminogenic needs.

Hourly Rate: $26.45

Schedule: Mon-Thurs 12pm-8pm & Fri 9a-5pm

Position Overview

  • Perform client needs assessments within seven (7) days of intake and enter data into CSI data systems on the day the data is obtained. Enter into funder database within five (5) days of client advancement into Services.
  • Actively participate in Client Progress Review (CPR) meetings by presenting new clients, assessments and Individual Service Plan updates on clients as well as discharge planning for clients nearing discharge. Provide and elicit peer support for client issues.
  • Develop and document program plan goals with clients within thirty (30) days of activation and review with client on a bi-weekly basis. Enter goals and relevant client data into Individual service Case Plan and ensure ARMS database reflects case planning efforts and real time progress. Monitor program stipulations, job readiness preparation, and community resource referrals.
  • Schedule, conduct, and document individual case management sessions that address client personal issues and program performance. Enter each session enter into CSI data systems on the day the data is obtained.
  • Case Managers are responsible for weekly curricula facilitation including:
    1. Entering all new referrals to interventions they facilitate into the CSI data systems on the day of the referral.
    2. Prepare, plan and facilitate individual and group sessions with integrity and fidelity to curriculum/manual.
    3. Schedule and conduct make-up sessions as needed.
    4. Documentation of group and individual sessions are to be entered into the agency data systems on the same day the information is received as well as funder database.
    5. Must maintain a minimum of 75% completion rate for each intervention facilitated.
  • Maintain a working knowledge of and abide by the Agency's programs, policies, procedures and code of ethics. Perform other tasks as assigned by supervisor.

Qualifications:

  • Bachelor's degree and one year experience in case management or similar field. Education may be substituted with experience on a year for year basis.
  • Excellent oral, written and interpersonal skills.
  • Ability to engage and develop collaborations with community agencies.
  • Previous assessment and facilitation experience preferred.
  • Experience with Microsoft Office.
