Role: LVN, Project Based Housing ODR

Reports to: Program Manager, Project-based Housing

Department: DHS, ODR

Location: Sherman Way, 13561 Sherman Way Blvd, Van Nuys

Schedule: Monday - Friday, 8:00 am - 4:30 pm

Status: Full Time, Non-Exempt

SUMMARY
The People Concern has partnered with the Department of Health Services (DHS) and the Office of Diversion and Re-entry to provide housing and Intensive Case Management Services (ICMS) to persons being released from jail with a prior history of homelessness. Participants are linked to the ODR program while they are in jail. Once released, Case Management staff will assists individuals matched to an ODR Project Based Housing site with the lease up process. Once housed, the goal is to help individuals with housing stability and retention by providing intensive services.
The LVN, ODR will function as part of a multi-disciplinary team, and provide medical interventions to individuals matched to Project Based Housing. The LVN will assume responsibility for addressing medication and health issues in the program and ensure that high quality nursing services is provided. These service include medication management, medical assessment, and follow up regarding medical appointments.

ESSENTIAL DUTIES & RESPONSIBILITIES
1. Work as part of an interdisciplinary team to provide comprehensive, integrated care to individuals experiencing homelessness, mental illness and substance addiction
2. Provide basic nursing services as needed, including injections, to clients residing in permanent supportive housing.
3. Remain current in knowledge of Medi-Cal benefits and healthcare systems navigation, which includes ability to enroll client's in Med-Cal
4. Coordinate PCP appointments or other medical follow up appointments
5. Provide medical case management/medication management to individuals including but not limited to medication monitoring, patient teaching, and supportive services to increase compliance with medication regimens
6. Along with Program Manager, maintain medication logs for clients receiving psychiatry services from The People Concern
7. Coordinate with pharmacy to ensure client medication is delivered on time
8. Document and chart all services provided in appropriate charting form with accuracy and in real time
9. Collaborate and communicate effectively with internal and external programs on behalf of clients to ensure continuity of care
10. Provide crisis intervention assistance when needed
11. Triage to appropriate community agency in an emergency (i.e., police, paramedics, fire department)
12. Maintain open communication with direct supervisor and leadership team
13. Participate in weekly staff meetings and weekly individual supervision
14. Ensure that medical services are reflective of the agency's treatment model, which emphasizes member choice, harm reduction, trauma-informed care, and psycho-social rehabilitation
15. Maintain confidentiality at all times per HIPAA and all applicable guidelines


QUALIFICATIONS
1. Current, California license as a Licensed Vocational Nurse with a clear license.
2. Minimum one years' experience working with individuals experiencing chronic homelessness, mental health, and substance abuse issues
3. Able to physically lift, move, and position members as needed.
4. Proficient in Microsoft Word, Outlook, Excel, and database applications.
5. Detail oriented with excellent time management, organizational, written, verbal, interpersonal, and computer skills.
6. Valid CA Driver's license, reliable car, auto insurance, and an acceptable driving record.
7. Able to obtain and maintain CPR/1st Aid certification.


WORK ENVIRONMENT
1. Office environment with occasional field activities
2. Regularly required to sit, stand, bend and occasionally lift or carry up to 35 pounds.
3. May be exposed to elements like cold, heat, dust, noise and odor.
4. May need to bend, stoop, twist, and sit throughout the day.
5. Will necessitate working in busy and at times loud environments
