Job Description

This full-time Social Worker works with Palliative Care patients at Santa Barbara Cottage hospital. The Medical Social Worker (MSW) is responsible for performing care coordination, resource linkage, clinical interventions, program development, utilization review, biopsychosocial assessments, and discharge planning for patients admitted to Cottage Health. The MSWs' primary focus is on assisting psychosocially complex patients as well as medically complex patients through their hospital stays to help facilitate successful discharges. MSW activities will result in quality outcomes, optimal care/cost management of services and/or procedures, a high level of customer satisfaction, and contribution to an overall value oriented experience of stakeholders and persons served.

QUALIFICATIONS
All job qualifications listed indicate the minimum level necessary to perform this job proficiently.

LEVEL OF EDUCATION
Minimum: Master's Degree in Social Work from a Council on Social Work Education (CSWE) accredited School of Social Work.
Preferred: LCSW is preferred

CERTIFICATIONS, LICENSES, REGISTRATIONS
Minimum: California Children Services (CCS) Paneling (within six months of hire)

YEARS OF RELATED WORK EXPERIENCE

Minimum:1-2 years of relevant clinical social work experience required

Preferred: 3-4 years of related experience in a healthcare setting preferred
